OverviewThis packet includes registration information as well as general information about the Central Santa Clara Valley Youth Soccer League (CVYSL) and other soccer organizations that your team may interact with. Note that most all of this information can be found on the Central Santa Clara Soccer League website – www.cvysl.org.
Teams affiliated with CVYSL may choose to register with one or more soccer associations under the FIFA umbrella.
The registration information included in this packet addresses in detail team registration with the California Youth Soccer Association – North (CYSA-N). CVYSL is a member of CYSA North, our State organization (Oregon border to Fresno). CYSA North is divided into districts, with CVYSL being part of District II (from Millbrae to Salinas/Monterey). Most team administrative matters are handled through the league and through the District II office of CYSA North.
Central Valley is also a member of US Club Soccer. Registration with US Club Soccer is a separate process from CYSA registration. This instruction packet provides information and links for some of the US Club Soccer playing leagues. For more information on registration with US Club Soccer, go to the website at

- Recreational (Division 4 or Class 4): Involves the majority of the players in the district. Players are assigned to teams on which all players must play at least 50% of the time throughout the season. Emphasis is on player development and soccer enjoyment.
- Developmental (Division 3 or Class 3): For those players who have a greater commitment to advancing their skills. Players compete in leagues that involve traveling and may participate in a number of tournaments. The Association Cup is a CYSA-N tournament for Division 3 teams.
- Select (Division 1 or Class 1): For players who are committed to the greatest advancement of their technical and tactical skills, as well as playing at one of the highest levels possible. Teams compete in traveling leagues and in local, state, regional, and/or national tournaments.
Registration of Division 1 and Division 3 teams and players with CYSA-N involves:
- registering for CYSA through your home league (CVYSL), referred to as “league of registration”
- registering for a playing league:
- Fall: Delgado, Abronzino, Premier, GSSL, etc.
- Spring: DII Spring League or US Club playing league – NorCal Premier or Super Y-League
Registration of a team with CVYSL normally takes place in the May - July time frame. USYSA and CYSA North operate on a seasonal year, running from August 1st – July 31st, with team and player registrations valid for this time period. The main exception to this registration calendar is new teams formed during Jan/Feb tryouts. Those teams will complete their CYSA registration in the Feb/March timeframe, with the registration valid only for the second half of the seasonal year (spring into summer). For a more complete understanding of registration procedures, read CYSA North Team Manual, Section 3, Section 4 and associated PIMs (Policy Interpretation Memoranda).
Registration of a team or players with US Club Soccer can be completed at any time. Registration is valid from August 1 of the current year through July 31 of the following year.
Team Approval ProcessTeams and coaches wishing to register with CVYSL must submit a team application in the late November/early December timeframe, and have those applications approved by the league. Only teams approved by the league will be registered. This application and approval process is repeated each year, even for teams already part of the league.
DII Coaching License PolicyHead coaches must submit a copy of their coaching license with their team application. All coaches and assistant coaches must submit a copy of the applicable coaching license to the league registrar with their registration form in order to be issued a coach’s pass, if the license isn’t already on file with the league registrar. The licensing requirements for DII coaches are as follows:
- All Div 1/C1 and Div 3/C 3 head coaches shall hold at least a CYSA E/D coaching license.
- All Div 1 and Div 3 assistant coaches shall hold at least a CYSA F coaching license.
- New coaches of teams U15 and younger must obtain a CYSA F license in their first year, an E license in the next year, and an E/D license in their third year if they desire to be a head coach of a Div 1 or Div 3 team in District II.
- All head coaches of Div 1 teams U16 or above in DII will be required to hold at least a CYSA State D coaching license or the equivalent NSCAA National diploma.
- No individual may serve as the head coach of more than three Div 1 and/or Div 3 teams at the same time.
Note that with the new LeagueOne registration system, coaching license information can only be entered into the database by the CYSA-N state office. Central Valley registration contacts should have received a list of current CVYSL coaches showing whose licenses are already entered in the system and whose are not. For coaches on that list who are not yet in the database and who had already submitted a copy of their license to CVYSL, those copies have been forwarded to the state office, and hopefully will be entered into the new database. For new coaches in CVYSL and coaches taking more advanced courses, it will be your responsibility to make sure your new license is recorded by the CYSA-N state office.
Registration Calendar and Playing League LinksIn addition to registering your team with CVYSL, you will need to register for a playing league. Each playing league has its own registration timeframes and requirements. It is your responsibility to understand and meet those requirements. Note that there are different playing leagues in the Fall and the Spring. Your best sources of information are the websites for the playing leagues. Note that any/all of the dates listed below could change. Please check the websites regularly for updates. If required by the playing league, be sure to turn in playing league rosters and questionnaires to the CVYSL registrar by the posted deadlines.
Fall Playing Leagues

Spring Playing Leagues
- February – on-line registration opens
- March – on-line registration closes – usually near the beginning of the month
- Refer to the NorCal website for application, regulations, deadlines and fees.
- Not registering for a formal playing league: some teams choose to continue practicing and playing scrimmages, but not commit to a formal playing league in the spring, mainly due to scheduling conflicts.
Managing CYSA Team PaperworkTeam BinderOnce you receive your completed team registration paperwork, you are ready to put together your team binder. All official paperwork in the binder should be kept in protective sleeves. Binders must be at the field with the coach and players at all practices and games. The 1601 USYSA Registration Forms are the medical release forms needed in case of an emergency. For teams planning to attend tournaments, binders should be ordered as follows:
- Goldenrod (roster) – must be most recent goldenrod, reflecting any adds/releases/transfers
- Player 1601 USYSA Registration Forms – these forms should be in the following order:
1st – players on the initial roster, in alphabetical order
2nd – players added/transferred to the team, in order of add/transfer
Never write on any of the “official” paperwork – goldenrod or passes. If there is an error, contact your league registrar.
Additional items you will probably want to keep in your binder include:
- Game Cards – you must supply a game card for home games. Game cards are distributed with equipment in August and additional game cards are available from the league registrar.
- Roster Stickers – template available on CYSA-N website. You must supply one of these to put on the game card at each game. If you choose not to make these, you will need to write in your roster on the game card for every game.
Note changes from previous years:
- Team Roster Change Forms – not kept by the team as part of their binder – changes to roster will be reflected on the new goldenrod you will receive at the time you process the roster change
- Player Proof of Age & Name – CYSA North states that these are no longer required for tournament check-in, as all birth date verification documents must be checked by the district registrar before passes can be issued, and therefore do not need to be checked again. However, it is strongly recommended that at least for the 2008-09 season, you have these documents with you at tournament check-in “just in case,” and especially for “out of state” tournaments (eg. SCal). If there is a foreign document translation (FDT) for the player, that should be kept with the copy of the birth certificate.
Player & Adult PassesEvery player and adult registered with your team will have a pass. Referees will enforce the rule “no pass, no play, no exception,” as the pass is proof of insurance. To ready your passes for game day:
- Separate the membership card from the pass.
- Note that player/coach signature is not required by CYSA-N, though there is still a place for the signature on passes.
- Laminate all player and adult passes (required by CYSA).
- Punch hole in corner of passes and put them on a ring in alphabetical order, with players and adults grouped separately. If you have adds or transfers, put passes on the ring in the chronological order in which players join the team (should match the order of your binder paperwork).
Procedural/Courtesy RemindersFor those of you who have been in this league for awhile, much of this may be “old news.” For newcomers, hopefully the information below will help make your assimilation into the league as smooth as possible.
- League Meetings: Attending monthly league meetings is the primary way to learn about what is going on in the league. League meetings also provide a great way for you to identify opportunities to be more involved in supporting the league. Please do not assume that anything you need to know will be sent to you directly. League meetings are third Tuesday of the month, 7:30pm. The location is posted monthly on the league website.
- Rain Days: Please remember to call the hotline if it is raining (or has been raining) in order to make sure the fields are still open:

- Team Registration Contact: Each team must have a registration contact. Please make sure the league registrar has the name and email address of this person. This is the person on your team who will receive any email updates regarding registration issues, etc.
- Contacting the Registrar (Angie Onate): Please use email – compreg@cvsyl.org. Email is checked regularly. Phone calls should be reserved for true emergencies or for league issues (vs team issues). If it is a true emergency or league business, then you are welcome to call. Angie will notify your team contact via email when your paperwork is ready.

- Paperwork: Always submit paperwork unfolded, not stapled, not taped, not glued. Use paperclips to hold things together. Use full size envelopes as required. Do not glue or tape envelopes shut as they will need to be opened and closed multiple times.
- Request for Forms: If you need forms, email your request. Note that there are days that the competitive registrar is not on email at all during the daytime so please plan ahead if you need forms.
Tournament Applications
CYSA North TournamentsApplications for CYSA N tournaments in the upcoming seasonal year (August thru July) can be sent out beginning May 1st. Applications cannot be postmarked before this date. For information on applying to tournaments, refer to:
Several notes about tournament applications:
- Applications should be sent directly by the team to the District II Tournament Coordinator. League registrar signature is not needed on tournament applications.
- Use only the appropriate year’s tournament application (2007/2008 application for 2007/2008 tournaments, 2008/2009 application for 2008/2009 tournaments, etc.).
Out of State Tournaments – Additional PaperworkIf the tournament you are applying to is outside of CYSA North (CYSA South or out of state), be sure you meet additional paperwork requirements for out of state tournaments as outlined below.
For tournaments outside of CYSA North, teams need to complete and process two additional forms:
1) Application to Travel
2) CYSA Official Travel Roster
Instructions for completing these forms are written on the forms. Note that these forms are due to the CYSA North state office no less than 30 days prior to the tournament. Refer to the websites listed above for detailed information.
Note that many teams are finding it simpler to attend tournaments (especially “out of state” tournaments) using US Club registration. You will need to check with each tournament to determine whether or not the tournament will accept teams using US Club registration.
Guest Players CYSA North teams may take guest players to tournaments, subject to any limitations imposed by the tournament. A
CYSA Guest Player Form #1611 must be completed for each guest player. A team may take no more than 18 players to a tournament. A guest player on a CYSA team must be a member of a team from any CYSA district.
This form requires the Parent/Guardian signature, the Loaning team coach signature, and the Borrowing team coach signature. The signatures of League and District officials are no longer required.
Team Registration for 2008-09 Seasonal Year

Team registration packets for the new seasonal year are usually distributed in May. If your team has been approved by CVYSL to play for the new seasonal year, please follow these instructions carefully to insure the quick and accurate processing of your team’s CYSA registration. In addition to completing this registration, you are responsible for registering your team for a playing league (see p. 3). To help ensure the viability of teams, CVYSL requires a minimum number of players in order to register a team. These requirements are:
- U10 and Younger: minimum of 10 rostered players, maximum of 13.
- U11 – U19: minimum of 13 rostered players, maximum of 18.
Note: If your team will be playing in a non-CYSA league (eg NorCal), the team must submit a Team Roster and applicable fees.
Submitting Paperwork
- Place all paperwork in a large envelope per instructions below.
- Write the following in the upper corner of the envelope (or use envelope with label):
- Team name
- Age group
- Playing league
- Date of 1st tournament using CYSA paperwork
- Registration contact name
- Registration contact email address
- I will assume that the date of your first tournament is when you need your paperwork
- Turn into league registrar.
- Assume four weeks for turnaround of completed team registration packets (2 weeks with league registrar and 2 weeks with district registrar).
Required Forms and Paperwork On all forms, please PRINT legibly. Also, please ensure that the parents on your team use only non-black pen, so as to distinguish originals from copies.
- Playing League Team Registration Forms: Printouts of on-line registration for Abronzino (D1), Delgado (D3), or other Playing League Team Registration Forms filled in on the playing league websites. These should be turned into the CVYSL Comp league registrar only if required by the playing league (historically required by Abronzino and Delgado; historically not required by Premier, GSSL).
- One check payable to CVYSL:
- Player registration fee of $130 per player.
- Adult Registration and Team Management Disclosure Form: pink, full size, multipart form. Must be completed for all adults working with the team -- coach, assistant coach, trainer, manager. Fields with an * must be completed in order to register.
- Copy of required coaching license, if not already on file with league registrar.
- Photo: Refer to details in Players section, below. Write name on the back of picture. Paperclip picture to Adult Registration and Team Management Disclosure Form.
- US Youth Soccer Membership Forms (1601s), in their own stack in alphabetical order.
- Proof of Age and Name: In a separate, alphabetized pile: legible copies of player birth certificates, passports, green cards, refugee cards, foreign birth certificates, or driver’s licenses. Any document that is not written in English must be accompanied by a completed Foreign Document Translation Form, approved by the District II Registrar. Please submit all copies on standard 8.5”x11” paper – do not cut down to the size of the document as small pieces of paper are more easily lost. Note that all US birth certificates must have a stamp from the county registrar to be acceptable (check the back of the original birth certificate if this stamp is not visible on the front).
- Photo:
- 1_”w x 1_”h photos of players, coaches, managers, cut to the correct size, with the name written on the back of each. Attach to the upper right hand corner of each player’s birth certificate using a paperclip, not staples.
- All photos must be a “face-shot” showing ears, hair, chin
- No hats or sunglasses
- No previously used player-pass pictures or otherwise laminated pictures.
- MAKE SURE PICTURES ARE CUT TO CORRECT SIZE, or they will be returned
FormsThe forms you will use most often are listed in the table below, along with a link to access the form on-line. You can also request forms from the league registrar. Following the table are instructions on how to complete the forms.
On all forms, please PRINT legibly. Also, please use only
non-black ballpoint pen, so as to distinguish originals from copies. On multi-part forms, please use
non-black ballpoint pen and press hard.
US Youth Soccer Membership Form (1601)This is a two-part form. If printing off the website, please turn in two copies. This form is also known as the medical release form and must be completed in order to register any player with CYSA.
Top Section – Fill in per instructions below:
- Season – 2008 – 2009
- League Name – Central Valley
- Age Group – UXX (U09, U15, etc.) – this is your Fall 2008 age group
- Div – leave blank
- Club/Team Name – Central Valley Sharks– should be complete team name as it will appear on your goldenrod. Team names must be unique. If there is another team in CVYSL with the same name as your team, then you must add the team year to the name, e.g. CV Sharks 95, or some other differentiator. If there are two teams with the same team name and team year, then you must add another differentiator such as color, e.g. CV Sharks Red 95.
- Region: 4B

- Team Number: 00XX – printed on the label on your team registration packet

- CYSA ID# – DO NOT fill in ID #s this year, as they are ALL CHANGING
Player information – must be completely filled in.
- Last name – must match last name on birth certificate/passport. Be careful with adopted children and remarriages! You need to use the exact name as it appears on the birth certificate to avoid delay in processing paperwork and potential ineligibility for tournaments. If last name is hyphenated and player only uses one name, it is okay to just put that one name.
- First name – must match name on birth certificate/passport. Can use either first or middle name as first name, but cannot use nicknames. If using two “first” names, must appear in same order as on birth certificate.
- Middle initial – you can leave this out, if you choose.
Family information – must be filled in, minus the sizing information
Liability Release and Medical Release – both must be filled in, signed and dated in both places by parent or by player if player is 18 or older. Note that if the player is 18 or older, the player must sign the form, not the parent.
Must have original (“wet”) signature in both places of the form in non-black ink. If using single page form printed from the web, must turn in two copies, both copies with original signatures in both places.
Foreign Document Translation FormIf the Proof of Age submitted by a player does not include an English translation as an official part of the document, a Foreign Document Translation Form must be completed and approved by the district registrar. When you submit this player’s registration, the FDT must accompany the player’s Proof of Age. FDTs can be printed off of the CYSA North or CYSA District II websites.
Adult Registration and Team Management Disclosure FormUsed to register all adults working with a team (coach, assistant coaches, trainers, manager, team assistants). All fields with an asterisk must be filled in or answered. Pink, full page, multi-part form. If printing the form off of the website, please turn in two copies.
Team Roster Change In Status Form – For Transfers and ReleasesThis form is required for all player transfers and for all player and adult releases. It is no longer required for player and adult adds, as these will be reflected directly on the goldenrod. The top part of the form not in a “box” must be filled in for all transfers and releases. Depending on which roster change you are making, complete the appropriate “box” in the center of the form. The bottom part of the form (signatures for accepting league and district) must be completed for all transfers and releases.
Making Changes to Your Roster
Player AddPlayers can be added to a roster throughout the seasonal year, subject to a 13 or 18 player maximum on the roster. Player is eligible to play the 8th day after the form is signed and dated by the district registrar (e.g. if signed and dated by district registrar on Friday, player is eligible to play Saturday a week later; 14 day wait required for Cup play). To add a player, turn in the following:
- US Youth Soccer Membership Form (1601)
- Proof of Age and Name
- Photo
- Required league and field fees
Adult AddAdults can be added to a roster throughout the seasonal year. To add an adult, turn in the following:
- Adult Registration and Team Management Disclosure Form
- Copy of required coaching license, if not already on file with league registrar
- Photo
Player TransferTeams are allowed a
maximum of three transfers on to their team during a seasonal year and unlimited transfers off of their team during a seasonal year (exception is U14-U19 State Cup). Player is eligible to play the 8th day after the form is signed and dated by the district registrar. To transfer a player onto a team, the following must be turned in to the
accepting league registrar
- Team Roster Changes in Status Form – requires signatures from parent, releasing team official and accepting team official. No longer requires league and district signatures.
- Accepting Team’s current Roster (goldenrod)
- Player pass
- New player photo
- $5 transfer fee (if check, make payable to CYSA District II)
Additionally, the
releasing team must turn in their Team Roster (goldenrod) to their league registrar. They will be issued a new goldenrod reflecting the transfer off of the player.
Player ReleaseAccording to the CYSA North Team Manual, “A player may be released from a team by the coach only if the player is unable to play for one of the following reasons:
- The player has violated USYSA, CYSA or League rules.
- The player has moved beyond a reasonable distance.
- The player is injured in such a manner that the player will not be able to participate for the remainder of the seasonal year.
- Dissolution of teams. A dissolved team can be defined as a team which has been registered and has been playing but ceases to exist before less than one-half (1/2) of their league season has been played.”
To release a player, the following must be turned in to the league registrar:
- Team Roster Changes in Status Form – requires signatures from parent and releasing team official.
- Releasing Team’s current Roster (goldenrod)
- Player pass

All paperwork should be turned in (not folded) in full size envelope with team name, age group, contact name, and contact email written in upper corner. Paperwork must be submitted to league registrar no later than Tuesday, 1 _ weeks before player expects to be eligible to play. Player is eligible to play the 8th day after the form is signed and dated by the district registrar (e.g. if signed and dated by district registrar on Friday, player is eligible to play Saturday a week later).
